«Saw VI» est le premier long métrage réalisé par Kevin Greutert, qui a assuré le montage de tous les films de la série. Greutert affirme que la transition du montage à la réalisation a été facile car il avait l'expérience de la réalisation de courts métrages et était familier avec les discussions avec les producteurs.

Aux premiers stades du scénario, une trame mettant en scène le détective Hoffman face à la mafia a été envisagée, mais a été rapidement rejetée car elle manquait de l'essence de «Saw» et ressemblait trop à «The Punisher».

Une semaine avant le début du tournage, Lions Gate a annoncé que le film serait converti en 3D, ce qui a bouleversé Greutert, qui avait conçu le projet en 2D. Ce plan de conversion 3D a finalement été abandonné en raison de contraintes de temps.

Dans l'émission de télé-réalité VH1 «Scream Queens» (diffusée en 2008), dix actrices inconnues ont conccouru pour le rôle du film, et Tanedra Howard, la gagnante, a obtenu le rôle de Simone. Elle a repris ce rôle dans le film suivant «Saw 3D».

Les scènes rétrospectives d'Amanda Young ont été nouvellement tournées pour ce film, plutôt que de réutiliser des images des films précédents comme cela avait été le cas jusqu'à présent.

Le réalisateur Greutert aurait aimé que le Dr Lawrence Gordon, interprété par Cary Elwes, réapparaisse dans ce film, mais l'emploi du temps d'Elwes ne l'a pas permis. Elwes a finalement joué ce rôle dans «Saw 3D», mais le déroulement s'est avéré différent de ce que Greutert avait initialement envisagé.

Le film a été produit avec un budget de 11 millions de dollars et tourné de mars à mai 2009 aux studios Cinespace Film à Toronto.

Le piège du manège a été conçu pour accueillir à l'origine dix acteurs, mais a finalement été réduit à six pour s'aligner avec le chiffre «6» du titre du film. Greutert décrit cette scène comme «la plus longue scène de piège» de toute la série.

L'Association américaine du film (MPAA) a demandé qu'une partie de la scène d'ouverture où Simone et Eddie se coupent la chair soit supprimée pour obtenir une classification R. En Espagne, le film a reçu une classification pour adultes en raison de la violence extrême, et le nombre de salles de cinéma montrant le film a été limité à huit.
