The series is a gender-flipped reboot of the 1989–93 ABC series Doogie Howser, M.D., developed by Kourtney Kang and put into development at Disney+ in April 2020.

The show is set in Hawaii specifically to reflect creator Kourtney Kang's own background.

Peyton Elizabeth Lee was cast in the titular role in January 2021, followed by Kathleen Rose Perkins, Jeffrey Bowyer-Chapman, and Jason Scott Lee as leads shortly after.

Milo Manheim joined the cast in a recurring role for the second season, playing a leukemia patient who becomes Doogie's new love interest.

Filming for the series began on December 7, 2020, in Honolulu and Los Angeles.

The lead character is nicknamed 'Doogie' as a direct nod to the original 1989 series Doogie Howser, M.D.

The show received an approval rating of 92% on Rotten Tomatoes based on 13 critic reviews, with critics praising how it paid homage to its predecessor while carving its own path.

Disney canceled the series in August 2023 after two seasons, despite it being renewed for a second season in February 2022.
