The series was directed by Ahn Pan-seok, known for Something in the Rain (2018) and One Spring Night (2019), which drew broadcaster attention to the project from the very start of production.

Jung Ryeo-won was offered the lead female role in July 2023, while Wi Ha-joon was offered his lead role three months later in October 2023.

Principal photography for the series began in October 2023.

The soundtrack was overseen by music director Lee Nam-yeon and featured the American band The Restless Age, with the reveal happening on May 10, 2024, just before the series premiere.

The full soundtrack album, containing all singles and background tracks from the show, was released on June 30, 2024, the same day as the series finale.

The series was originally planned as a Monday-Tuesday drama scheduled for around June, but due to protests over 2024 medical policy reforms, tvN scrapped its plan to air 'Best Physician, One Day' and instead confirmed this series as the follow-up to 'Queen of Tears'.

Starting from episode 5, the drama began offering an on-screen caption/subtitle service.
