Saw VI é o filme de estreia como diretor de Kevin Greutert, que havia trabalhado na edição de todos os filmes da série. Greutert afirma que a transição da edição para a direção foi fácil, pois tinha experiência em produção de curtas-metragens e estava familiarizado com discussões produtivas.

A armadilha do Carrossel foi inicialmente planejada para ter dez atores, mas foi reduzida para seis para corresponder ao número do título do filme, 'VI'. Greutert descreveu essa cena como a 'cena de armadilha mais longa' da série até então.

Filmagem

A Motion Picture Association of America (MPAA) solicitou cortes da cena inicial onde Simone e Eddie cortam sua própria carne para obter a classificação R. Na Espanha, o filme recebeu classificação para adultos devido à violência excessiva, com apenas 8 salas de cinema autorizadas a exibir o filme.
